Development and Implementation of an Interim Patient Care Report

This poster presentation will focus on the development and implementation of an Interim Patient Care Report. National Patient Safety Goal 2E, added for 2006, addresses the “implementation and standardization of an approach to ‘hand-off’ caregiver communication, including an opportunity to ask and respond to questions.”

This presentation will show how one institution adapted this goal to patients traveling between departments for procedures, utilizing this format to increase communication about patients. Process includes the multidisciplinary FOCUS–PDCA approach to the development, review, and implementation of this process.
